The underwater exploration company OceanGate publicly changed its procedures to reject established engineering codes and standards. They publicized they were going to innovate “like NASA” by developing a novel carbon fiber composite hull for its third submersible, the TITAN. The TITAN’s loss at sea with five people aboard in June 2023 captured worldwide media attention, which subsequently impacted the resulting investigations. This was the first loss of a non-military submersible in over 50 years and the first to be subject to a Marine Board of Investigation. The joint US Coast Guard/National Transportation Safety Board investigation brought much of the investigation process into the public, including the public hearings in 2024 and the publishing of the separate reports and findings of the USCG and NTSB in 2025. Kemper Engineering Services was the only non-governmental organization that was part of the official investigation. This presentation will be a summary of our findings, including the detailed failure analysis that indicates a second hull delamination between a 1-inch cured section is the most likely root cause of the implosion. We will also present our finding regarding design methods, simulation methods, acrylic window design, and air supply.
